The following Terms of Service apply to your use of these Self-Management Tools. You are solely responsible for your conduct and your content on the Self-Management Tools and compliance with these terms. By registering with us or using or browsing these Tools, you acknowledge that you have read, understood, and agree to be bound by these terms. These tools are not directed to children younger than 13 and are offered only to users 13 years of age or older. Any person who provides their personal information through these Tools represent that they are 13 years of age or older.

By using online self-management features on the Epilepsy Foundation website, such as WebEASE or CareEpilepsy, you agree to the following terms and conditions:

If you use these online management tools, you are the only User who can view and edit your information unless you choose to share your information with other Users. If you chose to share your information with others including caregivers, family or your clinicians, you can view a list of who has access to your information. Your provision of access to your personal information by others may result in third parties misusing your information and is at your own risk. At any time you can take away someone's ability to access your personal information and that person will no longer be able to read your health-related information, but they may have already seen or may retain a copy of the information.

You are responsible for taking all reasonable steps to ensure that no unauthorized person shall have access to your Epilepsy Foundation passwords or accounts and for choosing a username and password on the Epilepsy Foundation website that would not compromise security on any other website. For technical help with your self-management tools, you may have to provide your user name and password to the Site. You should not enter information into the self-management tools that would identify you personally.

You will not receive any payment or other reimbursement for use of the information entered into the self-management tools. If you choose to use the cell phone reminder program, which includes text messages (also referred to as SMS messages), you are responsible for all charges, including those assessed by your cell phone provider, incurred by you. Standard carrier rates may apply.

You agree that the Epilepsy Foundation may take action (including the removal of your content and disabling of your account) at any time in order to maintain compliance with these Terms and Conditions.

None of the self-management tools provide diagnosis, treatment or other medical advice. You should seek those from your doctors. If you are having a medical emergency, DO NOT use the self-management tools to communicate with your doctor.
